Зерттеудің мақсаты. Әзірбайжанда шашыраңқы склерозбен ауыратын пациенттерде диагноз қойылған сәттен бастап бақылау кезеңінің соңына дейін EDSS шкаласы бойынша анықталатын мүгедектіктің ұзақмерзімді өзгерістерін бағалау, сондай-ақ мүгедектік санаттары арасындағы ауысуларды және олардың ауру ұзақтығымен байланысын зерттеу.
Материалдар және әдістер. 2013 2022 жылдар аралығында ретроспективті және проспективті компоненттері бар ұзақмерзімді бақылаулық зерттеу жүргізілді. Талдауға диагноз қойылған кезде және зерттеудің соңында EDSS деректері бар, шашыраңқы склероз диагнозы расталған 238 пациент енгізілді. Мүгедектік жеңіл (EDSS 0 3,5), орташа (4,0 6,5) және ауыр (7,0 9,5) болып жіктелді. Уилкоксонның таңбалы рангтер критерийі, Пирсонның χ[2 ] критерийі, Крамердің V коэффициенті, Краскел Уоллис критерийі және Манн Уитнидің U-критерийі қолданылды.
Нәтижелер және талқылау. Диагноз қойылған кезде жеңіл мүгедектік 189 (79,4%) пациентте, орташа мүгедектік 49 (20,6%) пациентте анықталды. Зерттеудің соңында жеңіл мүгедектіктің үлесі 41,6%ға дейін төмендеді, орташа мүгедектіктің үлесі 43,3%-ға дейін артты, ал ауыр мүгедектік пациенттердің 15,1%-ында қалыптасты. EDSS медианасы 2,0 [1,0 3,0]-ден 4,3 [3,0 6,0]-ке дейін жоғарылады, ΔEDSS медианасы 2,5 [1,0 3,0] құрады (p<0,001). Бастапқы EDSS санаты мүгедектіктің соңғы деңгейімен тығыз байланысты болды (χ[2] (2)=104,935; p<0,001; Cramer’s V=0,664). Аурудың ұзаққа созылуы EDSS-тің анағұрлым ауыр санаттарына ауысумен байланысты болды.
Қорытындылар. Әзірбайжанда шашыраңқы склерозбен ауыратын пациенттерде EDSS бойынша мүгедектіктің елеулі прогрессиясы байқалды. Бастапқы EDSS және ауру ұзақтығы кейінгі мүгедектік қаупінің практикалық көрсеткіштері ретінде қарастырылуы мүмкін.
